Please follow these instructions when filling out a chain of custody record:<\/p>\n<ul>\n<li>Please type or print clearly.<\/li>\n<li>Please include the <span class=\"caps\">COURIER<\/span> (<span class=\"caps\">UPS<\/span>, Federal Express, hand delivered, etc.), <span class=\"caps\">METHOD<\/span> OF <span class=\"caps\">SHIPMENT<\/span> (Next Day Air, Ground, Priority Overnight, etc.), and <span class=\"caps\">TRACKING<\/span> <span class=\"caps\">NUMBER<\/span> in the spaces provided.<\/li>\n<li>Include the <span class=\"caps\">SAMPLE<\/span> ID on the form. The Sample ID should include the Claimant\u2019s Name, Claim Number, &amp; Date of Loss. (ex. Thomas Q. Public, Claim # 1234567, 1\/1\/00; object from cheeseburger).<\/li>\n<li>Include the <span class=\"caps\">SAMPLE<\/span> <span class=\"caps\">DESCRIPTION<\/span> on the form. The Sample Description should include the physical description of the actual components included.
